RESUMEN

The ultimate goal in implantology is to restore the whole tooth-gingival complex in a fashion that cannot be distinguished from the rest of the natural dentition. This study assesses the volumetric and clinical changes of vestibular gingival soft tissues, crucial for satisfactory engraftment and aesthetic results, upon treatment with laser-aided pouch roll augmentation in second-stage surgery for dental implant uncovering. Twelve patients with mild ridge deficiencies in 16 edentulous sites, including distal elements, were enrolled and re-evaluated for up to 1 year. Digital impressions, taken with an intra- oral laser scanner and software, were analyzed before (day 0) and after (month 12) the treatment. The digital STL files were superimposed to assess volumetric and linear dimensional variations in selected peri-implant regions of interest (ROI) by 3D analysis software. Clinical periodontal parameters (probing depth PD, bleeding on probing BoP, plaque index PI) and subjective patient-reported outcomes were also evaluated. In all the patients, the applied technique induced a substantial increase in the volume of the vestibular peri-implant gingiva at 12-month follow-up (range 24-69%, mean 40,4%) in respect to day 0. The gingival mucosa appeared normal at both inspection and evaluation of the periodontal parameters (PD 2.7±1 mm; BoP 0.11±0.2 s; PI 0.19±0). Patients' liking of perceived pain/discomfort and aesthetic outcome was high. These findings indicate that laser-aided pouch roll flap is a safe, patient-liked procedure whose long-term effectiveness has been objectively demonstrated by volumetric and clinical assessment.

BACKGROUND: Currently, the treatment of epulis is primarily surgical excision, which would greatly affect the aesthetics of patients if happened in the anterior region. It's challenging for clinicians to balance the aesthetic after surgery and less surgical trauma. To overcome this disadvantage, the authors propose the modified coronally advanced flap technique which applies the principles of minimally invasive surgery to provide satisfactory therapeutic results in fibrous epulis. METHODS: We report a case of an 18-year-old female with the chief complaint of a gingival swelling in the right upper anterior region. After the initial periodontal therapy, the modified surgical approach was applied to this patient. Unlike conventional coronally advanced flap technique, an additional incision was made, and the free portion was rotated into the adjacent space to completely cover the trauma, which avoided the use of the second operative zone. RESULTS: The gingiva recovered with normal color, contour, and consistency after surgery, the papilla filled up the proximal space well and was in good harmony with the adjacent papillae. The surgical results remained stable during the follow-up period. CONCLUSIONS: The use of modified coronally advanced flap technique allows the clinician to successfully resume the natural appearance of gingiva in the treatment of fibrous epulis, as well as simplify the surgical approach, shorten the operative time, and demonstrate no tendency of recurrence. KEY POINTS: Why is this case new information? This novel technique not only removes the epulis, but also takes into account the postoperative aesthetics of the surgery at the same time. This minimally invasive surgical technique reduces operative time and increases patient comfort. Keys to successful management of this case are as follows: (i) Adequate preoperative assessment of the location of the additional incision; (ii) tension-free coronal flap advancement. What are the primary limitations to success in this case? Clinical studies with long-term outcomes of this approach are needed. This procedure may be limited to larger gingival tumors.

OBJECTIVE: This clinical study aimed to evaluate the effect of virtual reality (VR) technology on anxiety and pain levels in patients undergoing gingivectomy and gingivoplasty procedures. MATERIALS AND METHODS: The patients were randomized into test (surgery with VR glasses) and control (surgery without VR glasses) groups. Before the surgery, the anxiety level was measured using the modified dental anxiety scale (MDAS), and anticipated pain (AP) was measured by a visual analog scale (VAS). Immediately after the surgery, the pain and discomfort associated with the surgery (VASP), procedure time (T), and time perception (TP) were evaluated. The patients in the test group were requested to assess the immersion (VASI), satisfaction (VASS), perception of reduced anxiety (VASA), and perceived control (VASC). One week after surgery, MDAS was applied to all patients. RESULTS: This trial was conducted with 41 female and 17 male subjects with a mean age of 29.69 ± 12.32. There were no significant differences between the groups in terms of age, sex, preoperative MDAS, or AP. After surgery, MDAS, VASP, T, and TP failed to differ significantly between the groups. The subject age was positively correlated with VASI, VASS, VASA, and VASC (r = 0.60, p = 0.00; r = 0.44, p = 0.02; r = 0.46, p = 0.02; r = 0.50, p = 0.01, respectively) and negatively correlated with VASP (r = 0.47, p = 0.04). CONCLUSIONS: VR application did not affect anxiety and pain levels in patients undergoing periodontal surgery. More studies are needed to evaluate VR distraction in periodontal surgeries with diverse age samples and video options. CLINICAL SIGNIFICANCE: VR does not affect anxiety and pain levels during gingivectomy and gingivoplasty surgeries in the young adult population. It should be evaluated in older age groups. The aetiology of gingival enlargement (GE) is highly distinct. Plaque-induced gingival inflammation can be the sole reason for gingival enlargement. Poor dental hygiene, irritation from anatomical variations, and ineffective restorative and orthodontic appliances are all factors that encourage the formation and retention of plaque. In the given case report, a case of gingival enlargement associated with an orthodontic appliance of a 23-year-old female patient referred from the Department of Orthodontics was reported to the Department of Periodontics. Under local anaesthesia, the excess gingival tissue is removed using a scalpel by gingivectomy and gingivoplasty procedures. The gingivectomy and gingivoplasty procedures using a scalpel gave the best results in the orthodontic treatment associated with gingival enlargement. After achieving hemostasis, the periopack (Coe-pack) was placed to assist healing by protecting the tissue. The above case report can appreciate the gingival tissue covering almost half of the crown, causing plaque retention and presenting the patient with aesthetic concerns. After the surgical procedure, a proper gingival contour eliminates suprabony pockets and provides pleasant esthetics. This case report demonstrates that eliminating the suprabony pockets by gingivectomy and gingivoplasty leads to a physiologic gingival contour and eliminates plaque retention. The conventional scalpel gingivectomy procedure is an effective form of treatment when indicated.

Sorriso gengival é comumente conhecido pela exposição excessiva das gengivas ao sorrir, acarretando assim na diminuição das coroas dos elementos. No que tange a etiologia dessa condição, ela se apresenta de maneiras diversas e sua identificação é fundamental para o desenvolvimento de um plano de tratamento eficaz. O planejamento clínico pode ser feito usando uma abordagem dinâmica e digital do sorriso, como softwares, exames de imagem, modelos virtuais 3D e guias cirúrgicos, melhorando a qualidade e precisão do tratamento, oferecendo diversos benefícios aos pacientes. Sendo assim esse estudo teve como objetivo demonstrar a importância dos processos digitais no planejamento e correção do sorriso gengival. Paciente queixou-se da desproporção do sorriso, caracterizado por coroas curtas nas regiões de pré-molares e incisivos ligadamente com a exposição excessiva da gengiva ao sorrir. Após estudos clínicos e de imagem, a etiologia foi diagnosticada como erupção passiva alterada, tipo I (fenótipo espesso). O tratamento de escolha foi o remodelamento gengival associado à osteotomia e osteoplastia obtendo assim uma maior precisão no tratamento, sendo confeccionado um guia cirúrgico duplo. Sendo assim, é possível contemplar que o planejamento digital permite uma maior previsibilidade da execução, compatibilidade em relação a expectativa do paciente e profissional, além da total individualização do caso, tornando os resultados mais favoráveis e exatos e minimizando as chances de iatrogenias(AU)


Gummy smile is commonly known for exposing the gums to the smile, thereby increasing the crowns elevation of the elements. Regarding a condition, it presents itself in different ways and its identification is fundamental for the development of a treatment plan. The clinician can be done using a dynamic and digital approach to image treatment, such as software, imaging exams, various 3D virtual models and elaborate guides, improving the quality and precision of treatment, offering benefits to patients. Therefore, this study aimed to demonstrate the importance of digital processes in planning and correcting the gummy smile. Patient complained of disproportion of the smile, facing the regions of premolar crowns and incisors of the smile, facing the exposure of the gingiva when smiling. After clinical and imaging exams, the diagnostic studies were diagnosed as passive eruption, type I (phenotype and specific). The choice of choice was remodeling associated with surgery treatment and surgery treatment, thus providing a greater precision in the treatment, being a double guide elaborated. A cirurgia plástica gengival tem sido cada vez mais procurada por pacientes que se apresentam descontentes com o seu sorriso. O procedimento convencional realizado é feito através de uma elevação de retalho para exposição de tecido ósseo e subsequente a osteotomia ou osteoplastia, o qual leva a necessidade de suturas e uma recuperação mais demorada. Com o avanço da tecnologia, a técnica cirurgia Flapless, conhecida como minimamente invasiva e indolor, onde trás uma recuperação mais favorável e com resultados satisfatórios. Tem como principal indicação para pacientes com biotipo periodontal fino e intermediário, o planejamento cirúrgico deve ser feito com exames complementares, como a tomografia, para poder ser calculado a quantidade de tecido ósseo a ser removido. Após a excisão do tecido gengival sem abertura de retalho, a osteotomia/osteoplastia é feito via sulco gengival com a utilização do ultrassom piezoelétrico. Evidencias clínicas e científicas mostram que a técnica Flapless possui resultados satisfatórios em um menor espaço de tempo, sem necessidade de suturas e melhor pós-operatório.


Gingival plastic surgery has been increasingly sought after by patients who are dissatisfied with their smile. The conventional procedure performed is done through a flap elevation to expose bone tissue and subsequent osteotomy or osteoplasty, which leads to the need for sutures and a longer recovery. With the advancement of technology, the Flapless surgery technique, known as minimally invasive and painless, brings a more favorable recovery and satisfactory results. Its main indication for patients with thin and intermediate periodontal biotype, surgical planning should be done with complementary exams, such as tomography, in order to calculate the amount of bone tissue to be removed. After excision of the gingival tissue without opening a flap, the osteotomy/osteoplasty is performed via the gingival sulcus using piezoelectric ultrasound. Clinical and scientific evidence shows that the Flapless technique has satisfactory results in a shorter period of time, without the need for sutures and better postoperative period.


La cirugía plástica gingival ha sido cada vez más solicitada por pacientes que no están satisfechos con su sonrisa. El procedimiento convencional que se realiza se realiza mediante una elevación del colgajo para exponer el tejido óseo y posterior osteotomía u osteoplastia, lo que conlleva la necesidad de suturas y una recuperación más prolongada. Con el avance de la tecnología, la técnica de cirugía Flapless, conocida como mínimamente invasiva e indolora, trae una recuperación más favorable y resultados satisfactorios. Su principal indicación para pacientes con biotipo periodontal delgado e intermedio, la planificación quirúrgica debe hacerse con exámenes complementarios, como la tomografía, para calcular la cantidad de tejido óseo a remover. Después de la escisión del tejido gingival sin abrir un colgajo, la osteotomía/osteoplastia se realiza a través del surco gingival mediante ultrasonido piezoeléctrico. La evidencia clínica y científica demuestra que la técnica Flapless tiene resultados satisfactorios en menor tiempo, sin necesidad de suturas y con mejor postoperatorio.

Objective @#To evaluate the effect of anatomical healing abutments for mandibular first molars on the morphological changes of gingival soft tissue after implant restoration, as well as on peri-implant gingival molding, food ingrowth and patient satisfaction, to provide a basis for clinical selection. @*Methods @# Twenty-six patients who received implant restoration for a single missing mandibular first molar between September 2020 and September 2021 at the Oral Implant Center of Changsha Stomatological Hospital were randomly divided into a control group (13 cases with 14 implants) and a trial group (13 cases with 14 implants), of which 12 were male and 14 were female; the trial group had anatomical healing abutments applied for 4 weeks and then underwent crown restoration, while the control group finished five time points (before the second stage surgery, 4 weeks after the second stage surgery, immediately after the crown restoration, 4 weeks after the crown restoration, and 12 weeks after the crown restoration). A 3Shape intraoral scanner was used to scan the jaw before and 4 weeks after the second stage surgery to quantify the soft tissue changes and compare the effect of the healing abutment on gingival molding between the two groups. The incidence of food impaction was recorded and compared between the two groups at 4 weeks and 12 weeks after crown restoration. Patient satisfaction was recorded and compared between the two groups immediately after crown restoration, 4 weeks after crown restoration and 12 weeks after crown restoration@*Results @# Four weeks after implant surgery, it was observed that the gingival proximal and distal gingival papillae increased on the coronal side in the test group compared to the control group, 0.50 (0.26, 0.72) mm in the near-medium test group and 0.27 (0.24, 0.38) mm in the control group, with a statistically significant difference (P = 0.029), and 0.48 (0.26, 0.62) mm in the far-medium test group and 0.23 (0.13, 0.39) mm in the control group, with a statistically significant difference (P = 0.004). There was no statistically significant difference (P>0.05) in the buccolingual to gingival margin apex to coronal molding or in the buccolingual to lingual soft tissue at 0, 1, or 2 mm of the root of the middle 1/3 apex of the buccal and lingual gingival margins between the two groups. Compared to the control group, there was no statistically significant difference in the incidence of food impaction observed 4 weeks and 12 weeks after crown restoration in the test group (P>0.05). The satisfaction scores were higher in the trial group than in the control group immediately, 4 weeks, and 12 weeks after crown restoration, and the difference between the groups was statistically significant (P < 0.05). @*Conclusion @# The anatomical healing abutment for the implant mandibular first molar was superior to the finished healing abutment in terms of soft tissue contouring with an increase in the coronal aspect of the proximal and distal gingival papillae, resulting in high patient satisfaction.

BACKGROUND: Hereditary gingival fibromatosis (HGF) is characterized by sub-epithelial fibromatosis of keratinized gingiva resulting in a fibrotic enlargement of keratinized gingiva. The treatment choice is gingivectomy, which can be performed with an internal or external bevel incision conventionally. However, both techniques can hardly resume the natural status of gingiva, and have a certain recurrence rate, especially in the cases which have limited width of attached gingiva. CASE DESCRIPTION: Two cases of HGF with the chief complaint of difficulty in mastication, pronunciation, and poor esthetics were presented. After the initial periodontal therapy, a novel gingivoplasty modified with a crevicular incision was applied. A full thickness flap above the mucogingival junction and a split flap below the junction were raised. Then, fibrotic connective tissue was completely eliminated and keratinized gingival epithelium was preserved. The fibrotic alveolar bone was shaped by handpiece and bur. Finally, the flap was apically repositioned and sutured. Twelve months after surgery, the gingiva recovered with normal color, contour and consistency. CONCLUSIONS: Compared to traditional gingivectomy, modified gingivoplasty which focuses on eliminating pathological fibrotic connective tissue can completely resume the natural appearance of gingiva and demonstrate no tendency of recurrence.

Contexto:O cimento cirúrgico é um material alternativo no pós operatório de gengivoplastia e gengivectomia, que proporciona conforto ao paciente. Porém, existem controvérsias sobre sua utilização, sendo relatado por alguns especialistas o surgimento do processo inflamatório agudo, sendo classificado como desnecessário.Objetivo: Identificar evidências na literatura acerca do efeito do cimento cirúrgico no pós-operatório do tecido gengival após gengivectomia e gengivoplastia. Materiais e métodos: Foi realizada uma revisão crítica de literatura por meio de buscas nas bases de dadosPubmed, Scielo, Lilacs, Cochrane, Scopus e na literatura cinzenta (Periódicos Capes, Google Scholar). Foram incluídos ensaios clínicos randomizados que testaram o cimento cirúrgico em casos de gengivectomias. Os artigos foram selecionados inicialmente pelo título e resumo, seguido de análise do texto na íntegra. Os estudos incluídos tiveram o nível de evidência avaliado por parâmetros do CONSORT. Resultados:Foram levantados 1458 estudos, sendo 16 escolhidos para leitura na íntegra e, destes, 3 foram incluídos na revisão. A amostra teve idade variando de dezessete a setenta e um anos. Foi utilizado cimento cirúrgico sem eugenol comparado com o bochecho de clorexidina 0,2% e a escovação mecânica precoce no pós-cirúrgico. Um estudo relatou menor índice de dor com o uso de cimento cirúrgico. O tempo de acompanhamento dos estudos foi de 30 dias. Dois estudos tiveram nível de evidência III.Conclusão: Os artigos apresentaram baixa evidência científica e quenão há consenso na literatura quanto ao efeito de cimento cirúrgico após gengivectomias.


Aim:To identify evidence in the literature about the effect of periodontal dressing on the healing of gingival tissue after gingivectomy and gingivoplasty. Materials and methods: A critical literature review was carried out through electronic searches in thePubmed, Scielo, Lilacs, Cochrane, Scopus databases and in the gray literature (Periódicos Capes, Google Scholar). Randomized clinical trials evaluating periodontal dressing in gingivectomies were included. The articles were selected by title and abstract,followed by analysis of the full text. The included studies have the level of evidence determined by CONSORT parameters. Results: A total of 1458 studies were surveyed, 16 were selected for full reading and, of these, 3 were included in the review. The age sample ranged from seventeen to seventy-one years old. A periodontal dressing without eugenol was used compared to a 0.2% chlorhexidine mouthwash and an early mechanical brushing after surgery. One study reported less pain with the use of periodontal dressing. The follow-up of the studies was 30 days. Two studies had Level of Evidence III.Conclusion:It was concluded that the articles had low scientific evidence and that there is no consensus in the literature regarding the effect of periodontal dressing after gingivectomies.

Dental aesthetics are a fundamental treatment goal in dentistry, in which even minute deviations from the ideal may necessitate corrective treatment or constitute a suboptimal clinical outcome. A well-defined protocol that adheres to sound biological and surgical principles is necessary to harmoniously integrate the dental and periodontal components. This article reviews clinical and aesthetic guidelines based on these principles for clinical crown lengthening.

This case report is aimed to demonstrate the synergetic effects of λ940 nm laser photobiomodulation (PBM) therapy in augmenting the advantages of high-level-laser treatment (HLLT)-mediated reaction orthodontic periodontal interface management. Materials and Methods: A 32-year-old female who presented with a persistent gummy smile of upper incisors and low upper midline frenum attachment post-orthodontic treatment, was seeking a better smile appearance. She had a history of delayed wound healing without underlying medical conditions; otherwise, she was fit and healthy. She underwent laser ablation of the upper midline frenum and gingivoplasty of the upper incisors region with λ940 nm and λ2780, respectively, as well as transcutaneous PBM therapy (λ940 nm) to accelerate wound healing. The laser protocols were as follows: λ2780 nm: power output-2 W, pulse width-60 µs, free running pulse (FRP), spot area-0.0016 cm2, pulse repetition rate-25 pulses per second (s), 80 mJ/pulse, 90 s, λ940 nm: 1.2 W, continuous wave (CW) emission mode, 300 µm, 60 s; whereas the adjunctive λ940 nm induced-PBM parameters were as follows: power output-1.4 W, CW-120 s, single application, spot area-2.8 cm2. An acceleration of the wound healing was observed on the 4th day of treatment with no immediate or post-operative complications. The results showed no functional or aesthetic relapses at a long-term follow-up of 6 months. The authors concluded that λ940 nm laser-PBM can provide a synergetic effect to HLLT in accelerating wound healing and offering a precision smile with minimal to none post-operative complications. It is safe and justifiable to utilise dual therapy over the conventional methods, which serves our patients' needs in our daily practice and in various clinical indications. The concept and laser protocols of this clinical case report can pave the roadmap for future extensive studies.

OBJECTIVE: The aim of this study was to evaluate the effect of platelet-rich fibrin (PRF), concentrated growth factors (CGF), and autologous fibrin glue (AFG) application on early wound healing after gingivectomy and gingivoplasty operations. METHOD AND MATERIALS: In this split-mouth study, gingivectomy and gingivoplasty surgery were performed on 19 patients. The postoperative PRF, CGF, and AFG applied areas were compared with the control regions. On days 0, 7, 14, and 28, the surgical area was stained with a plaque-disclosing agent and evaluated in the ImageJ program. Wound healing was evaluated with H2O2 test, visual analog scale for pain, and Landry, Turnbull, and Howley (LTH) wound healing index on days 7, 14, and 28. The patients were asked to evaluate their esthetic perceptions on a visual analog scale. RESULTS: The amount of staining at days 7 and 14 was found to be significantly higher in the control group than in the test groups, but there was no difference between the test groups. LTH index values of the control group at days 7, 14, and 28 were found to be significantly lower than the test groups. There was no significant difference between the groups in the epithelialization assessment performed with the H2O2 test. It was observed that the use of platelet concentrate at day 7 reduced postoperative early pain. Patients were highly satisfied with postoperative esthetics. CONCLUSION: After gingivectomy and gingivoplasty operations, PRF, CGF, and AFG application were found to have positive effects on wound healing. However, PRF, CGF, and AFG applications were not superior to each other in terms of secondary wound healing.

PURPOSE: In this study, we aimed to evaluate the clinical validity of the modified tunneling technique using minimal soft tissue harvesting and volume-stable collagen matrix in the anterior mandible. METHODS: In total, 27 anterior mandibular teeth and palatal donor sites in 17 patients with ≥1 mm of gingival recession (GR) were analyzed before and after root coverage. For the recipient sites, vertical vestibular incisions were made in the interdental area and a subperiosteal tunnel was created with an elevator. After both sides of the marginal gingiva were tied to one another, a prepared connective tissue graft and volume-stable collagen matrix were inserted through the vestibular vertical incision and were fixed with resorbable suture material. The root coverage results of the recipient site were measured at baseline (T0), 3 weeks (T3), 12 weeks (T12), and the latest visit (Tl). For palatal donor sites, a free gingival graft from a pre-decided area avoiding the main trunk of the greater palatine artery was harvested using a prefabricated surgical template at a depth of 2 mm after de-epithelization using a rotating bur. In each patient, the clinical and volumetric changes at the donor sites between T0 and T3 were measured. RESULTS: During an average follow-up of 14.5 months, teeth with denuded root lengths of 1-3 mm (n=12), 3-6 mm (n=11), and >6 mm (n=2) achieved root coverage of 97.01%±7.65%, 86.70%±5.66%, and 82.53%±1.39%, respectively. Miller classification I (n=12), II (n=10), and III (n=3) teeth showed mean coverage rates of 97.01%±7.65%, 86.91%±5.90%, and 83.19%±1.62%, respectively. At the donor sites, an average defect depth of 1.41 mm (70.5%) recovered in 3 weeks, and the wounds were epithelized completely in all cases. CONCLUSIONS: The modified tunneling technique in this study is a promising treatment modality for overcoming GR in the anterior mandible.

Introduction: Hereditary gingival fibromatosis is a rare disorder with a genetic component that may appear during tooth replacement. This condition can cause functional and aesthetic problems such as malocclusions, diastemas, pain when chewing, dental caries, periodontal disease, delayed eruption, among others. Objective: To report the multidisciplinary treatment provided to a patient with hereditary gingival fibromatosis. Case Report: This report describes the treatment carried out in a thirteen-year-old male patient presenting generalized increase in gingival volume associated with functional and aesthetic compromise and delayed eruption of permanent teeth. After diagnosis, a multidisciplinary intervention was proposed, involving periodontal and pediatric dentistry procedures, which improved the quality of life of the patient both functionally and aesthetically. Conclusion: Hereditary gingival fibromatosis not only affects the dental eruption process, but also causes aesthetic and emotional alterations in the patient. The periodontal procedures significantly improved the appearance, function, and the psychological state of the patient.


Introducción: La fibromatosis gingival hereditaria es una altera-ción poco común, asociada a un componente genético que en ocasiones se hace evidente en el recambio dentario. Este padecimiento puede generar problemas funcionales y estéticos como maloclusiones, diastemas, dolor al masticar, caries, enfermedad periodontal, erupción tardía, entre otros. Objetivo: Reportar el caso clínico con manejo interdisciplinario en un paciente con fibromatosis gingival hereditaria. Reporte de Caso: Se expone el tratamiento realizado en un paciente de trece años, sexo masculino, con aumento de volumen gingival generalizado con compromiso funcional y estético, conjugado con retraso en la erupción de dientes permanentes. Tras diagnóstico se plantea la intervención multidisciplinaria, integrando áreas como periodoncia y odontopediatría; los procedimientos ejecutados permitieron mejorar la calidad de vida desde el punto de vista funcional y estético. Conclusión: La fibromatosis gingival hereditaria no solo desencadena alteración en proceso eruptivo dental, sino también alteraciones estéticas y emocionales en el paciente que la padece. Los procedimientos perio-dontales realizados permitieron de forma categórica la mejora de la estética, función y estado psicológico del paciente.

Procedimentos estéticos odontológicos estão cada vez mais integrados com outras áreas, trabalhando em prol da otimização de resultados psicossociais e funcionais, e aumentando as possibilidades inclusas no plano de tratamento. A estética apresenta grande relevância na odontologia moderna, a demanda por sorrisos brancos e harmoniosos aumenta proporcionalmente às novas técnicas existentes. O presente trabalho objetiva relatar um caso de reabilitação estética a partir do planejamento digital do sorriso que dará informações benéficas para avaliar as diversas possibilidades frente ao segmento restaurador e periodontal cirúrgico. Paciente R. C. C., 22 anos, sexo masculino, compareceu a clínica integrada com queixa estética nos dentes anteriores. A partir do exame clínico inicial, foram solicitados exames de rotina concomitantemente à moldagem para modelo de estudo. Após o estudo de caso, esclarecimento do planejamento ao paciente e frente as expectativas estéticas, foi solicitado o exame de Planejamento Digital do Sorriso (Digital Smile Design ­ DSD). Foi realizada a cirurgia de gengivoplastia e osteoplastia dos pré- -molares nos dentes 12, 14, 15, 24 e 25, fundamentada nos parâmetros fornecidos pelo exame digital e restaurações diretas em resina composta após o período de cicatrização adequado. Cada vez mais, é exigido do cirurgião-dentista, uma visão ampla que permita explorar as diversas perspectivas vigentes atendendo aos interesses e necessidades do paciente. Conclui-se, independentemente da área da atuação, a realização e otimização de resultados caminha lado a lado com a multidisciplinariedade que, a cada dia que se passa, ganha importância na formação acadêmica profissional.


Aesthetic dental procedures are increasingly integrated with other areas, working towards the optimization of psychosocial and functional outcomes, and increasing the possibilities included in the treatment plan. Aesthetics has great relevance in modern dentistry, the demand for harmonious white smiles increases in proportion to new techniques. The present abstract aims to report a case of aesthetic rehabilitation from the digital smile planning that will give beneficial information to evaluate the various possibilities before the restorative segment and surgical periodontal. Patient R. C. C., 22 years old, male, attended the integrated clinic with aesthetic complaint in the anterior teeth. From the initial clinical examination, routine exams were requested concomitantly with the modeling of the study model. After the case study, clarification of patient planning and aesthetic expectations, the Digital Smile Design (DSD) exam was requested. Gingival surgery was performed on teeth allied to osteoplasty of the premolars 12, 14, 15, 24 and 25, based on the parameters provided by the digital examination and direct restorations in composite resin after the appropriate healing period. Increasingly, the dental surgeon is required to have a broad vision that allows exploring the diverse perspectives that are in place, taking into account the interests and needs of the patient. We can conclude that, regardless of the area of performance, the realization and optimization of results goes hand in hand with the multidisciplinarity that, every day that passes, gains importance in the professional academic formation.

Introdução: Os padrões de beleza atuais aumentaram o nível de exigência estética e a busca por sorrisos cada vez mais harmônicos. As facetas cerâmicas destacam-se dentre as opções de tratamentos por associarem estética, longevidade e previsibilidade. Objetivo: Este trabalho objetivou apresentar um caso clínico de substituição de facetas diretas de resina composta por facetas indiretas de porcelana. Descrição do caso: A pacienteC.L.M, 25 anos, sexo feminino relatou insatisfação com a estética do seu sorriso, destacando o formato, o tamanho e a cor 1M2dos elementos dentários 11 e 21. Diante da queixa e da situação clínica observada, propôs-se a confecção de laminados cerâmicos de dissilicato de lítio, mediante a realização prévia de gengivoplastia e de clareamento dentário. Conclusões: Conclui-se que o planejamento cuidadoso, o conhecimento da técnica operatória e a execução adequada do protocolo reabilitador permitem que os laminados cerâmicos reestabeleçam o sorriso de forma estética e funcional (AU).


Introduction:The current beauty standards have increased the level of aesthetic demand and the search for more and more harmonious smiles. The ceramicveneers stand out among the treatment options for combining aesthetics, longevity and predictability. Objective:This study aimed to present a clinical case of substitution of direct resin veneers composed of indirect porcelain veneers. Case description:Patient C.L.M, 25 years old, female, reported dissatisfaction with the aesthetics of her smile, highlighting the shape, size and 1M2 colorof dental elements 11 and 21. In view of the complaint and the clinical situation observed, it was proposed to make lithium disilicate ceramic veneers, through previous gingivoplasty and tooth bleaching. Conclusions:It is concluded that the careful planning, the knowledge of the operative technique and the proper execution of the rehabilitation protocol allow the ceramic veneers to reestablish the smile in an aesthetic and functional way (AU).


Introducción: los estándares de belleza actuales han aumentado el nivel de demanda estética y la búsqueda de sonrisas cada vez más armoniosas. Las facetas cerámicas se destacan entre las opciones de tratamiento para combinar estética, longevidad y previsibilidad. Objetivo: Este estudio tuvo como objetivo presentar un caso clínico de sustitución de carillas de resina directas compuestas de carillas de porcelana indirectas.Descripción del caso: La paciente C.L.M, 25 años, mujer, refirió insatisfacción con la estética de su sonrisa, destacando la forma, tamaño y color 1M2de los elementosdentales 11 y 21. En vista de la queja y la situación clínica observada, se propuso hacer laminados de cerámica disilicato de litio, mediante reparación gingival previa y blanqueamiento dental. Conclusiones: se concluye que la planificación cuidadosa, el conocimiento de la técnica operativa y la ejecución adecuada del protocolo de rehabilitación permiten a los laminados cerámicos restablecer la sonrisa de una manera estética y funcional (AU).

ntrodução:Pacientes com dentes curtos e ameias abertas, relatam muita insatisfação com seusorriso.A gengivoplastia, quando bem indicada, permite oaumento no tamanho da coroa dentária de forma a reduzir a necessidade de aumento incisal com resina composta. A reanatomização com resinas compostas para fechamento de ameias e aumentosincisais representa uma alternativa conservadora, pois trata-se de uma técnicaadesiva diretaque pode ser realizada sem nenhum desgaste dentário. Além disso é uma técnica reversível preferível para pacientes jovens. Objetivo:Apresentar um relato de caso clínico de paciente do sexo masculino, 22 anos, insatisfeito com o tamanho, cor e a forma dos seus dentes anteriores. Descrição do Caso:Ao exame clínico foi observado coroa curta, escurecimento fisiológico, espaços interdentais e desalinhamento das bordas incisais. O plano de tratamento proposto foi encaminhar para a gengivoplastia, clareamento dental pela técnica associada e reanatomização estética dos dentes anteriores com resinacomposta diretaestratificada com diferentescamadas e cores, e posterior texturização superficialdurante as fases de acabamento e polimento finais. Conclusões:Com a gengivoplastiae asrestaurações, obtivemos dentes reanatomizados, reproduzindo naturalidadede forma, textura e cor. Com planejamento e técnica adequada, foi possível restabelecer o sorriso comproporções, forma e naturalidade satisfatórias (AU).


Introduction:Patients with short teeth and open battlements, report a lot of dissatisfaction with their smile. Gingivoplasty, when properly indicated, allows for an increase in the size of the dental crown in order to reduce the need for incisal augmentation with composite resin. Resuscitation with composite resins for the closure of niches and incisal increases represents a conservative alternative, as it is a direct adhesive technique that can be performed without any dental wear. Furthermore, it is a preferable reversible technique for young patients.Objective:To present a case report of a 22-year-old male patient, dissatisfied with the size, color and shape of his anterior teeth.Case Description:The clinical examination showed a short crown, physiological darkening, interdental spaces and misalignment of the incisal edges. The proposed treatment plan was to refer to gingivoplasty, tooth whitening by the associated technique and aesthetic reanatomization of the anterior teeth with stratified direct composite resin with different layers and colors, and later surface texturing during the final polishing and finishing phases. Conclusions:With gingivoplastyand restorations, we obtained reanatomized teeth, reproducing natural form, texture and color. With proper planning and technique, it was possible to restore the smile with satisfactory proportions, shape and naturalness (AU).

Objective: To evaluate the efficiency of Laser Low Intensity (LLI) in pain control and reduction of analgesic consumption after gingivectomy and gingivoplasty. Methods: Twenty patients with gingival hyperplasia were randomly divided into two groups with 10 subjects each. In the test group it was applied after gingivectomy / gingivaplasty, low intensity laser in the operative wound and prescription only in case of pain, while in the control group only analgesia was performed in case of pain. Results: The laser was applied at 3 points in the region of periodontal surgery. The irradiation was performed in a single session, using diode laser (808 nm, 100nW, 100J / cm², 30 s). Pain was assessed by Visual Analogue Scale, as well as the amount of analgesics ingested by the patient. In the test group, 80% of the individuals did not report postoperative pain, those who reported symptomatology, were identified only as mild discomfort. The analysis of the data showed a statistically significant difference when compared to the higher postoperative pain between the groups (p = 0.026). However, there was no difference in the evaluation of analgesic use. Conclusion: Based on our results, it is suggested that LLI therapy decreases the intensity of postoperative pain in periodontal surgeries (AU)

RESUMEN

This clinical case report describes a conservative approach to a smile harmony rehabilitation performing a gingivoplasty, dental bleaching and resin-based composites to finalize the case. This clinical article describes the case report of a patient in need of an esthetic and corrective solution for the anterior teeth, which presented a disharmonic smile, in its shape and color, besides the gingival smile. As a result, gingivoplasty, dental bleaching and restoration with composite resin were done. Gingivoplasty with bone preservation was possible because there was a large volume of soft tissue, and a distance ≥ 2 mm from the bone crest to the junction cementum enamel was confirmed. Bleaching was performed with the use of associated techniques: office (Whiteness HP 35%) and supervised at-home (Whiteness Perfect 10%) bleaching were performed, and then the "buccolingual technique" using a nanocomposite resin IPS was used from a diagnostic wax-up to perform the restorative procedure for the anterior teeth. The use of minimally invasive techniques is an effective alternative for the anterior teeth rehabilitation. As in this case, the gingivoplasty, dental bleaching with associated techniques, and restorations with nanocomposites allowed satisfactory esthetic results, even more so in relation to dental preservation, mainly in young patients. Este relato de caso clínico descreve uma abordagem conservadora de uma reabilitação para harmonia do sorriso que foi realizado com gengivoplastia, clareamento dentário e resinas compostas para finalizar o caso. Este artigo clínico descreve o relato de caso de uma paciente com necessidade de solução estética e corretiva para os dentes anteriores, que apresentava um sorriso desarmônico, em sua forma e cor além do sorriso gengival. Como resultado, gengivoplastia, clareamento dental e restauração com resina composta foram realizados. A gengivoplastia com preservação óssea foi possível por apresentar grande volume de tecidos moles e uma distância ≥ 2 mm, desde a crista óssea até a junção do cemento esmalte. Clareamento com o uso de técnicas associadas: Clareamento de consultório (Whiteness HP 35%) e caseiro supervisionado (Whiteness Perfect 10%) foram realizados, e depois, a "técnica da barreira palatina" utilizando a resina IPS Empress direta foi realizada a partir de um enceramento diagnóstico para iniciar o procedimento restaurador dos dentes anteriores. O uso de técnicas minimamente invasivas é uma alternativa eficaz para reabilitação de dentes anteriores. Como no caso, a gengivoplastia, o clareamento dental com técnicas associadas e restaurações com nanocompósitos têm permitido resultados estéticos satisfatórios, ainda mais em relação à preservação dentária, principalmente em pacientes jovens. The esthetic excellence has become the main objective in the dental treatment. The gummy smile is one of the complaints from the patients, since this situation may influence their self-esteem, social relationship and quality of life. The development of new techniques search to be therapeutic option more conservative, in the treatment of gummy smile, such as the association between the gingivoplasty and application of botulinum toxin, in comparation with other surgical procedures as the orthognatic surgery. The purpose of this article is to present a case of a patient with dentogingival discrepancy and gummy smile, who was treated with the association of these techniques, in order to optimize the harmony of the smile


A excelência estética tornou-se o principal objetivo no tratamento odontológico. O sorriso gengival é uma das queixas dos pacientes, uma vez que esta situação pode influenciar a sua autoestima, relação social e qualidade de vida. O desenvolvimento de novas técnicas busca ser uma opção terapêutica mais conservadora, no tratamento do sorriso gengival, como a associação entre a gengivoplastia e a aplicação da toxina botulínica, em comparação com outros procedimentos cirúrgicos como a cirurgia ortognática. O objetivo deste artigo é apresentar um caso de uma paciente com discrepância dentogengival e sorriso gengival, que foi tratada com a associação dessas técnicas, a fim de otimizar a harmonia do sorriso.
